Here are answers to some commonly asked questions. If you do not find the answer you're searching for, please call us, 913-588-2454.

Question: How do I get a copy of my key medical record information?
Answer:

Ask your clinical site to assist you and give you a document that explains MyChart

MyChart gives you secure online access to portions of your electronic medical record. Some features of MyChart include:

  • The ability to see your future appointments
  • What your care team recorded about recent visits
  • Your medications and immunizations
  • Most lab results  
  • Inpatient results

 Question: How do I get a copy of my medical record?
 Answer:

If you need more than is accessible in MyChart, the Health Information Management Department will be happy to assist you. Your written authorization is required. Download the authorization form now.

Choose from the following:

  1. Ask your clinical area to provide you with the authorization form.
  2. Download the authorization form. Download the authorization form in Spanish. 
  3. Send an e-mail request to roi@kumc.edu.
    We can then reply by attaching the authorization form for you to complete and return.
  4. Call Health Information Management at 913-588-2454.
  • Important:
    • Do you need your medical records within 24 hours?
      If you must have your records within 24 hours and you are at the Main Hospital campus, you may go to the following location: Basement Level, Room B430.  The sign will say Health Information Management. Keep in mind this is only when you need records within 24 hours.The hours we are open are 8:30 a.m.–5 p.m. Monday through Friday.

    • Are you an inpatient about to be discharged?
      If you are an inpatient and just being discharged, keep in mind that we need a few days for your record to be available and are not able to provide the copy at the time of discharge.
       
    • Where do you pick up medical records?
      Copies of records may be picked up at our main office:

      Mission, KS  66202

      Hours: Weekdays, 8:30 a.m.–5 p.m.

      When records are picked up, the authorization must name the specific person  to whom you want your records released. Anyone picking up copies of medical records will be asked to show identification.

Question: Is there a charge or fee for copies of medical records?
 Answer:

We do have a fee for copies of medical records. If we send medical records directly to a physician or healthcare provider, there is no fee. If we provide the medical records to the patient, there is a fee.

 Question: How much is the fee?
 Answer:

The fee is based upon the number of pages. The Health Information Management Department will provide that information based upon what is specifically being requested. More detail can be found on the instructions for completing the authorization (refer to No. 7).

 Question: How long will it take to receive medical records requested?
 Answer:

Our average turnaround time for continuing care requests is about one business day. This can vary depending upon critical need, if the record is complete at the time of the request, and if there is a need to obtain a paper chart.
